The responses to questions 1 thru 7 range from 0 to 5 points, with 0 points for "not at all" and 5 points for "almost always." Once you have answered questions 1 thru 7, add together the point values of your answers and compare your total score to the symptom severity range provided below.

Question 8 is a condition specific quality of life question meant to assess how you feel about your present condition.

3. INTERMITTENCY

During the last month or so, how often have you stopped and started again several times when you urinated?

4. URGENCY

During the last month or so, how often have you found it difficult to postpone urination?

5. WEAK STREAM

During the last month or so, how often have you had a weak urinary stream?

6. STRAINING

During the last month or so, how often have you had to push or strain to begin urination?

7. Nocturia

During the last month, how many times did you most typically get up to urinate from the time you went to bed at night until the time you got up in the morning?

Scoring Scale

Mild:	0 to 7
Moderate:	8 to 19
Severe:	20 to 35
